If there is someone sick at home or in the hospital. call the priest so that the sick person, still conscious, can receive the sacrament of confession, annointing of the sick and communion.
If the sick person is not conscious, call the priest to give to the sick the anointing of the sick or also called "Last Rites".
Please do not leave everything to the last, your loved one needs spiritual assistance, do something for her/him on time and do not wait until your loved one is dying beause there may not be a priest available to help him/her at the last moment.

The anointing of the sick is administered to bring spiritual and even physical strength during an illness and can be received more than once, especially near the time of death. It is most likely one of the last sacraments one will receive. A sacrament is an outward sign established by Jesus Christ to confer inward grace. In more basic terms, it is a rite that is performed to convey God’s grace to the recipient, through the power of the Holy Spirit.
The sacrament can be received by any Catholic whose health is in a critical state oir in danger of death.
